NATURE OF WORK

Under general supervision, promotes community awareness of developmental disabilities services; presents resource education materials and outreach services; conducts educational forums and events to promote access to relevant services.

ESSENTIAL FUNCTIONS:

The following duties are the fundamental, crucial job duties performed by this position. This is not a comprehensive list of all tasks that may be assigned. Incumbent may be required to perform other duties as assigned and appropriate for the position.

• Oversees and coordinates outreach and community information regarding activities throughout Lewis County in compliance with Washington State Developmental Disabilities Community Services program standards.

• Plans, organizes, and conducts community outreach programs, community events, workshops, conferences and discussion groups to promote access to developmental disability and other applicable supports and services.

• Educates the public on developmental disability resources and community services available through Public Health & Social Services Department and other community agencies.

• Prepares public exhibits, and distributes brochures, flyers, posters, and other materials for resource promotion activities to the public, community agencies, and support groups.

• Provides education and resource navigation to clients and their families, social contacts, and the community; documents client information and provides referrals to medical and community services agencies.

• Coordinates, facilitates, and/or participates in meetings with developmental disability and transition partners.

• Set ups and maintains program files on clients, assuring client confidentiality at all times; assists clients with community program application process.

• Prepares and delivers presentations to potentially eligible clients on assigned topics. • Documents client service delivery in program reports as required. • Assists in compiling and maintaining records, reports and statistical information.

• Maintains absolute confidentiality of work-related issues, personnel records and County information; complies with the confidentiality standards of the Privacy Act of 1974, {U.S.C. § 552A} as amended, and HIPAA policies and procedures.

WORKING ENVIRONMENT / PHYSICAL DEMANDS:

Work is performed in a standard office environment and off-site environments, including community locations; work involves light physical demands and frequent use of a personal computer.

QUALIFICATIONS:

REQUIRED: Education, experience, certifications, etc.

• High School Diploma or G.E.D. equivalent; AND four (4) years' experience in a disability or social services program.

• A valid Driver's License.

• Specific technical training and certifications may be required. PREFERRED: Education, experience, certifications, etc. COMPETENCIES (Knowledge, Skills, Abilities, and Behaviors): Knowledge of:

• Development Disabilities, Division of Vocational Rehabilitation programs and DDA best practices.

• School based transition and supported employment services.

• Applicable laws and regulations covering social service and disability programs.

• Community service agencies and other governmental assistance programs.

• Principles of record keeping and records management.

• Business and personal computers, and standard software applications.

Skills in:

• Assessing community needs, providing outreach and advocacy services, and making eligibility

recommendations based on observations and interviews.

• Presenting information to potentially eligible clients and the public.

• Facilitating stakeholder meetings

• Interacting with people of all social, economic, cultural, and ethnic backgrounds.

Maintaining accurate records.
