MANAGEMENT APPRAISAL OF PERFORMANCE PLAN (MAPP): This position is subject to the provisions of the Management Appraisal of Performance Plan (MAAP). Initial salary placement and subsequent salary adjustments will be made in accordance with MAPP guidelines and regulations.

WHAT DOES A DIRECTOR, CARE TRANSITIONS DO?: Oversees the development and administration of transitional care services within the jail health system of care. This includes managing the jail's discharge planning and community linkage process.
Essential Job Functions SOME RESPONSIBILITIES OF A DIRECTOR, CARE TRANSITIONS INCLUDE: Provides program and administrative direction to subordinate program managers and administrative support staff.
Directs the conduct of, and participates in, studies, audits, and analyses of the jail discharge, reentry, and linkage effectiveness; and initiates modifications to policies and procedures as necessary toward process improvements while ensuring Department of Justice (DOJ) compliance and audit requirements.
Assists the Correctional Health Director in the development and implementation of objectives, goals, policies, and procedures in connection with areas of oversight; including but not limited to substance use treatment programs, Medicaid Waiver programs and other Medi Cal funding streams to support transitional care services
Collaborates with patients/inmates, healthcare team, Sheriff's Department, and the Probation Department to develop a comprehensive a discharge process that enhances patient/inmate satisfaction, efficiency of resources, and improved post-discharge outcomes.
Provides or oversees the provision of programmatic and technical guidance to health related contract agencies; directs the planning, monitoring and evaluation of contract agency performance.
Represents jail health services in planning/networking efforts (e.g., Department of Justice Advisory Councils, councils of government, regional health, social, housing, and educational services coalitions).
Assists in the development of appropriate quality improvement processes, including establishing and monitoring indicators.
Coordinates the delivery of staff training necessary to maintain mandated levels of services.
Oversees the preparation of monthly status reports including data requests, spreadsheets and graphs; Utilizes audit data and performance metrics to identify inefficiencies and drive continuous improvement.
Consults and collaborates with service providers and staff to support and develop processes to assist with reducing transition gaps and reentry.
Assesses and evaluates new admissions/discharges and determines if appropriate continuing care linkages were made.
Assists in the development and management of a health care transmittal system which allows for improved communication, coordination, and sharing of information within and outside the Jail Health Services system.
Ensures client care information is transmitted appropriately within the County and to community providers.
Requirements REQUIREMENTS TO QUALIFY: Option I: A valid, current license* to practice as a clinical social worker, marriage and family therapist, psychologist, or registered nurse issued by the appropriate State of California licensing agency - AND - three years of experience overseeing the provision of acute or case management services** in a comprehensive health care setting.
Option II: A Master's degree*** from an accredited college or university in Public Health or Public Administration - AND - five years of experience overseeing the provision of acute or case management services** in a comprehensive health care setting.
LICENSE: A valid California Class C Driver License or the ability to utilize an alternative method of transportation when needed to carry out job-related essential functions.
PHYSICAL CLASS II - Light: This class includes administrative positions requiring light physical effort that may include occasional light lifting to a 10 pound limit and some bending, stopping, or squatting. Considerable ambulation may be involved.

Additional Information EXAMINATION CONTENT: This examination will consist of an evaluation of experience, education and desirable qualifications based on application information, weighted 100%.
CANDIDATES ACHIEVING A PASSING SCORE OF 70% OR HIGHER ON THIS EXAMINATION WILL BE ADDED TO THE ELIGIBLE REGISTER. ELIGIBILITY INFORMATION: Applications will be processed as they are received and added to the eligible register accordingly.
