What You'll Be Doing

We are currently seeking a Housing - Social Worker to become a member of an interdisciplinary community services team for the HOPWA (Housing Opportunities for Persons With AIDS) population. Housing Affordability & Community Revitalization PHSS follows the Housing First Model.

The HOPWA Housing Social Worker utilizes a trauma informed framework to assess client’s needs and collaborates with the client to determine the best plan of action to meet these needs. The Social Worker utilizes Motivational Interviewing, Harm Reduction, and other clinical interventions to assist the client with maintaining health and housing stability. The Social Worker facilitates and coordinates access to the services and resources necessary for long-term health and housing stabilization. Services may be provided in the client’s home, field and/or agency and in some cases may require on-call availability.

The essential functions of the HOPWA Housing - Social Worker are as follows:

• Providing intensive case management; conducting home visits and engaging client in determining their personal living plan and updates in accordance with the client’s needs at least one visit per month
• Understanding the client’s health and mental health issues, treatment, medications, symptoms, side effects, impacts to health, housing stability and overall well-being
• Providing therapeutic services including symptom monitoring, supportive counseling, crisis stabilization and de-escalation, solution-focused counseling and other interventions as needed and appropriate; Role requires assessment, facilitation, coordination and monitoring the delivery of individualized services
• Negotiating with landlords regarding late payments, damage or lease violations and consulting with legal services staff regarding actions to prevent evictions. Assisting with rehousing if necessary
• Assisting clients with accessing community resources and services, such as job placement and training, debt counseling, legal aid, housing, medical and mental health or other treatment services, financial assistance and arranging or providing transportation as needed
• Maintaining required data and HMIS documentation and developing individual service plans for all clients
• Collaboration with internal and external partners to support collaboration in care planning and consultation to support clients

As the population and economy continue to grow in Wake County, the need for affordable housing development and preservation increases. Not only do more units of affordable housing need to be built, but appropriate services must also be provided to support families and individuals with both accessing and maintaining safe, decent, affordable housing. Affordable housing is also critical to preserving Wake County’s economic competitiveness by offering housing for workers of all income levels, supporting housing stability and economic opportunity for residents and furthering Wake County’s commitment to healthy and inclusive growth. The Department’s programs include: Community Revitalization, Development Financing, Homeless and Prevention Services, Emergency Shelter, Support Services, Veterans Services and Permanent Supportive Housing.

The Basics (Required Education and Experience)

• Bachelor's degree in Social Work or related field
• One year of job-related experience
• Valid Driver's License and "safe" driving record
• Equivalent education and experience are accepted

Beyond the Basics (Preferred Education and Experience)

• Three (3) years of work experience with case management services, housing, homeless, HIV and special needs populations
• Licenses/Certifications; LCSW - License Clinical Social Worker, CPR Certification - Cardiac Pulmonary Resuscitation (within 60 days) and First Aid Certification (within 60 days)
• Bilingual in Spanish

• Excellent written and verbal communication skills required to consistently, regularly, effectively and clearly communicate with Samaritan House constituents of diverse backgrounds and in various arenas of business and states of tension with all constituents
• Ability to take initiative and work independently to successfully collaborate with teams and present information in small or large group settings
• Ability to identify and resolve issues without direction when needed and take a solutions-based approach to all business needs

Emergency Service Worker Statement

In the event of an emergency, as determined by the County Manager or designee, participation in preparedness and response operations should be expected. Employee may be required to fill a temporary assignment in a role different from standard duties, work hours and/or work location in preparation for, during and after the emergency. Employee may also be required to participate in relevant exercises and regular preparedness training.

Salary:

23.94 - 40.70 Commensurate with Experience
